Sandwell Community Health Network, known as CHN, is a public health programme targeted at the Black and Minority Ethnic (BME) communities in Sandwell.
CHN currently works with 7 different community groups including:
• African Caribbean • Bangladeshi • Indian • Irish • Pakistani • Sikh • Yemeni
Our aims and objectives
• Improve and promote access to services and appropriate health information
• Empower and work with communities to identify their own health and wellbeing needs and find effective solutions
• Develop culturally appropriate health and social care programmes to reduce health inequalities
• Undertake community development and engagement activities
• Raise awareness of health and wellbeing issues
• Develop and manage community based health projects and deliver structured education programmes in community languages e.g. on diabetes, coronary heart disease and cancer
• Provide management support and link with community based workers
• Encourage social justice, community unity and inclusion of ethnic minority issues to mainstream policy making

• Women’s only empowerment and health activities
• Men’s only activities and elderly men’s groups
• Gardening projects
• Structured health education programmes

• Advice and help with accessing other services
