This is a referral scheme where your GP or practice nurse can refer your for physical activity advice and support to help with a medical condition you may suffer from. Working closely with Sandwell GP’s the programme has been built up over the year to include wide range of opportunities for people.
Once referred patients are given an appointment with one of our GP Exercise Referral Instructors. During the appointment the instructors discuss the patients current and previous lifestyle, medical conditions, feelings about exercise, and carries out various health checks (e.g. blood pressure, height and weight, waist hip ratio, resting heart rate). From this a physical activity action plan is specifically designed for each individual patient. This could be centre based (gym, swimming, classes) or EXTEND (gentle exercise classes), health walks, Walk to Beijing, gardening, green gym, simple lifestyle changes to exercise that could be done in the home.
This programme is given over a 3 month period after which every patient returns for a re-assessment and further advice and support. This programme is ideal for people who need advice and support if they suffer from a medical condition that can be controlled / improved by taking up more exercise.
